Milano, settembre 1493. Leonardo da Vinci è chiamato a indagare su un cadavere sospetto.
Durante l'autopsia, però, accade qualcosa di sconvolgente: la salma si rianima e assale lo studioso e gli assistenti. Scampato all'attacco, Leonardo viene convocato da Ludovico il Moro. Il potente signore di Milano lo mette a parte di notizie agghiaccianti: ciò a cui ha assistito non è che l'ultimo di una serie di casi che stanno terrorizzando l'Europa.
Il duca gli affida allora una missione: andare a Roma per informare papa Borgia e coordinare le forze per contrastare ciò che sembra essere una feroce e letale pestilenza. Leonardo intraprende così un viaggio durante il quale dovrà fare i conti con una realtà deformata: i morti sono tornati sulla terra e hanno fame, fame di carne umana.
A Firenze subirà gli assalti della follia di Savonarola e stringerà amicizia con Michelangelo, prima di ripartire per la Città Eterna, dove altre difficili prove lo attendono. Per risalire alla inconfessabile verità e porre rimedio all'inferno scatenato.
